| NRS 450 - Complex Disease Case Management |

This course focuses on care outcomes for clients with complex diseases across the lifespan. Learners incorporate theories from nursing and other disciplines to achieve perspective and understanding of the patients with complex disease experiences and the cultural implications for nursing practice. The course examines a holistic approach to assessing the needs of the patient and their families during end-of-life care. Learners will utilize tools and resources commonly used by care managers to assess individual patient care needs, develop a plan of care, and coordinate care. The student will emphasize the importance of effective communication with interdisciplinary teams and advocacy for patient individual needs.
