Apply standardized assessment tools and clinical guidelines in identifying patient needs and priorities 3. Advocacy for patient-centered care and shared decision-making within interdisciplinary settings 4. Develop individualized care plans based on comprehensive assessments and evidence-based practice guidelines 5. Identify supportive care services, including counseling, spiritual care, and community resources 6.
